- Origin: Copied by Muḥammad ibn ʻAbd Allāh ibn al-Mujill al-Baghdādī at al-Madrasah al-Niẓāmīyah in Baghdad, completed A.H. Safar 590 (Jan.-Feb. 1194, f. 86r).
Binding: Early calf over paper boards, blind-stamped with central four-lobed medallion and rolled double frame with text in the outer frame.
Collation: Paper, i + 86 + i; 1⁸ 2¹² 3⁸ 4¹² 5⁸ 6¹² 7⁸ 8¹² 9⁶. Catchwords on each leaf, lower left verso.
Script: Written in naskh in black ink; partially pointed.
