Extent: 12 leaves : paper ; 188 x 134 (145 x 98) mm bound to 190 x 139 mm.

Title from title page (fol. 1a).
After the colophon (fol. 8a) and on the following page and four folios are several notes (samʻ, ijāzah) about reading Kitāb al-Shifā and the names of those who heard it read. Some notes are dated in the 670s/1270s.
Collation: Paper ; fol. 12 ; modern foliation in pencil using Western numerals.
Layout: 19 lines per page.
Description: Paper tinted red ; lacking many iʻjām diacritic marks ; MS in good condition.
Origin: Perhaps from the XIVth century? Date discrepancy between the author's dates, and the dates from the reading notes.
