Ms. codex. Title from title page (front flyleaf recto), written in later pencil; badly faded and difficult to read. Incipit (f. 2r): ANIMA: Anima est actus primus corporis. Foliation: Paper, i (former pastedown) + 394 + i (former pastedown); 1-202, [stubs of 203-209], 210, 212, 215-352, 355-372, [stub of 373], 374-375, 377-400; contemporary foliation in ink, upper right recto. On many folios the wearing and fraying of the edges has obliterated the folio numbers. Folios 203-209 and 373 have been cut out, leaving stubs with traces of text. Folio 210 has also become... Script: Written in a cursive script by what appears to be a single hand. Binding: Contemporary limp vellum. Vellum of cover badly split, cracked and worn. Many leaves badly stained; possible water and mildew damage. Many leaves worn and fraying at the edges. Some oxidation of ink.
