Date from colophon (f. 87r), according to which the ms. was written by Paulus de Gnathi for Abbot Raphael of Czerwinsk, canon of the cathedral church of Plozk (Płock) in Poland. Text in Latin, written in large littera bastarda in one column in black, rubrics in red, major initial letters in black, blue, and red with some pen-work decoration, minor initials daubed in yellow. Proper prefaces and some other texts are set to music, the notation being a 4-line staff in red with horseshoe-nail-like notes. Bound in contemporary blind-tooled calf with 2 clasp catches. In a tray case, 35 cm. "Biblioteka Tulezynska" (stamps on ff. 1, 2) -- Jos de Witte (signature on f. 1r) -- Sophie Potocka (signature on f. 1r). Deposited by Philip Hofer, 1967; bequeathed, 1984.
